This ECG is diagnostic of Arrhythmogenic Right Ventricular Dysplasia.

The diagnostic features being:

  1. T inversion in V1 to V4;
  2. Right axis deviation;
  3. Epsiolon wave, best seen in lead V1; and
  4. Borderline prolongation of QRS duration

The palpitations he had was because of ventricular tachycardia which had a classic left bundle branch block and left axis deviation. This is suggestive of VT origin from the right ventricular apex. These patients have fat deposition replacing the ventricular myocardium, most often in the right ventricle causing ventricular tachycardia and right ventricular dysfunction and failure.
